Learning outcomes
The student is able to
- recognise the needs of a high-risk patient who needs a large number of services in an outpatient care
- assess the factors affecting the client’s capability to perform self-care and the realisation of self-care, and give guidance to the client
- take advantage of communal resources and digital services to support self-care
- develop guidance directed to the needs of the different client groups in primary health care
recognise the most common infectious diseases
- plan, implement and evaluate nursing of patients with long-term illnesses and infections and its impact in primary health care
- perform the most common examinations and treatment procedures during the patient’s appointment with the nurse
- coordinate the overall care of patients with long-term illnesses and infections in multidisciplinary cooperation in primary health care
- evaluate and develop service path for patients with long-term illnesses and infections from a client-centered point of view
