Learning outcomes
Student is able to
- identify the clients with multiple needs and with a high-risk of developing chronical conditions in outpatient care
- evaluate the factors influencing the client's self-management
- support self-management of the clients
- utilize and evaluate digitals services in supporting self-management of clients
- develop evidence-based guidance based on the needs of primary health care customer
- identify the most common infectious diseases in primary health care setting
- plan, implement and evaluate the care of chronically ill patients and the care of with patients with infectious diseases in primary health care
- manage and assess the medical treatment for chronically ill patients
- coordinate the nursing care of chronically ill patients in multi-professional cooperation in primary health care setting
- evaluate the service paths of chronically ill patients in primary health care setting
