Learning outcomes

The student is able to:
- evaluate a patient’s transition readiness (ABCDE) before and during the transition
- report systematically and clearly in the transition care process (ISBAR)
- provide patient education when discharging and transferring a patient
- take patient involvement into consideration and promote it in transitional care process
- evaluate and suggest improvements in the implementation and utilization of simulation pedagogy
- assess own professional development and identify areas for development regarding the topics above
